The provider I want to send to doesn't appear on the list?

After completing all reasons for referral and clicking ‘Next’, users will be required to select the patient’s choice of provider from a locally agreed directory of services (DOS). Please follow these steps if users are unable to locate a provider or unsure why a provider is not appearing:

  • Check the entire list using the scroll functionality on the right hand side (providers are listed in order of proximity to the patient’s home address)
  • In certain cases, patients may select a choice of hospital that is not available on the list. For further information please click this link
Please note:

The options available will be based on the information provided within the medical history and patient modifying factors section along with the patient's demographics (including age and location of registered GP practice), together with the level of clinical complexity for each reason for referral.

Some providers are restricted to patients who are registered with practices within specific Clinical Commissioning Groups (CCGs).
