Patient Choice Section

After completing all reasons for referral and selecting ‘Next’, users will be required to select the patient’s choice of provider from a locally agreed directory of services (DOS). Further information is provided below: 

  • Click  ‘Select’ on the right hand side of each provider (the selected provider will appear above the list in blue text)
  • In order to expedite patient care, it may be required to also select an option from ‘Provider If considered more appropriate following clinical review’ list 
Please note:

The options available in the Patient Choice section will be based on the information provided within the medical history and patient modifying factors section along with the patient's demographics (including age and location of registered GP practice), together with the level of clinical complexity for each reason for referral.

Some providers are restricted to patients who are registered with practices within specific Clinical Commissioning Groups (CCGs).

The referral will be sent to the choice of provider within the ‘Preferred provider (indicative triage decision)’ list first, and may forwarded on to the provider within the ‘Provider If considered more appropriate following clinical review’ list if required.

Providers are listed in order of proximity to the patient’s home address. To help inform the choice, the system also presents:

  • The distance of each provider from the patients home
  • Directions to the location
  • Waiting list information (where available).
