After indicating whether the patient has a severe gag reflex and/or severe dental anxiety along with any associated questions in the medical history and other patient modifying factors section, users will be requested to indicate whether they ‘Would you like to add/upload any additional information?’. Additional information is specific to the patient medical history and associated modifying factors; details relating to reason for a referral/treatment will be requested at a later stage as will the ability to upload radiographs and images.

If ‘Upload Information’ is selected, users will be unable to proceed (‘Next’) without uploading a document/image or deselecting the option and replacing with ‘None’ or ‘Free-text information’.
