How to add a Reason/s for Referral

After completing the medical history and patient modifying factors section and selecting ‘Next’, you will be required to select reason/s for referral from a drop-down list. Further information is provided below:

Adding reason/s for referral after selecting ‘Next’ from the medical history and patient modifying factors section:
- Type a reason for referral within the drop-down list provided; a smart search function will help identify the reason to select. Users may add as many reasons as required
- Click the ‘Next’ button to proceed or the ‘Edit’ button to return to the medical history and patient modifying factors section and amend any information provided
Once all reason/s for referral have been completed in the sections provided a message prompts users to either click the ‘Next’ button to proceed to the patient choice section, add additional reason/s, or amend any information provided following the steps above.

Please note:

The options available when selecting reason/s for referral will be based on the information provided within the medical history and patient modifying factors section along with the patient's demographics (including age and location of registered GP practice). It is therefore essential that all information provided is accurate.

Significant alterations to the medical history and patient modifying factors section may automatically deselect a reason for referral that has already been completed.

Each reason for referral selected will appear sequentially and clearly labelled.
