It is important to indicate any current or relevant medical history and other patient modifying factors. The complexity of conditions, medications and other factors may impact patient care and therefore the logic within the system; in some instances, prompts will offer further advice.

Once ALL required fields have been entered (including upload of any information if selected), a 'Next' button will appear.

By selecting 'Next' a list of possible reasons for referral will be presented based on the information provided within this section along with the patient's demographics (including age and location of registered GP practice), in accordance with locally agreed pathways. the information provided will also impact on the directory of services/list of providers that the patient may choose from.

With this in mind, and to ensure full accuracy, please treat this section with extra care and enter from the drop down boxes wherever possible as the logic on the system does not recognise information that has been uploaded or provided within free-text boxes.
